The use of ozone and low-intensive laser irradiation in complex treatment of complicated duodenal ulcer.

The study is based on the examination of 12 patients with perforative duodenal ulcer and 24 patients operated on recurrent bleeding duodenal ulcer. Some component of the immune system, such as T- and B-lymphocytes rates, immunoglobulin rate and macrophagal activity, were decreased prior the beginning of the complex treatment. Normalisation of humoral and cell immunity was registered on 10-12 days after the beginning of the ozone and low-intensive laser irradiation.

Application of low-intensive laser irradiation in the treatment of perforative gastroduodenal ulcers

Comparative analysis of results of surgical treatment, performed for perforative gastroduodenal ulcers in the patients, lacking H. pylori (control group) and owing this etiologic factor (main group), was conducted. Together with other conventional schemes of medicinal treatment in the early postoperative period a low-intensive irradiation of He-Ne laser was applied. Complete eradication of H. pylori was noted in 95.8% patients of the main group. In a late follow-up period (in 1-5 yrs observation terms) the frequency of recurrence, hemorrhage, pyloroduodenal stenosis and other surgical complications, occurring in the main group, was lesser than in a control one.

The laser chromoendoscopy eradication of Helicobacter pylori

35 patients with absence of positive dynamics after course of treatment of diseases associated with Helicobacter pylori infection and without achievement of eradication are surveyed. The original technique of Helicobacter pylori endoscopy eradication with use of the therapeutic laser in length of a wave of 660 nanometers is offered. Carrying out of rate laser chromoendoscopy eradication considerably reduces terms of healing of inflammatory-destructive changes of stomach and duodenum mucous membrane allows achieving of Helicobacter pylori eradication.

Efficacy of different laser treatments in combined therapy of patients with gastroduodenal ulcer

AIM: To investigate efficacy of different methods of low-intensity laser treatment (LILT) in therapy of exacerbated gastroduodenal ulcer.

MATERIAL AND METHODS: A total of 111 gastroduodenal ulcer (GDU) patients were divided into two groups: the study group (81 patients) and control group (30 patients). Patients from the study group received combined treatment with anti-ulcer drugs and LILT performed by three methods: intravenous laser radiation of blood (ILRB), epicutaneous radiation (ER) and combination ILRB+ER. Control patients received medication alone. The efficacy of the treatment was assessed by the time of ulcer defect heeling, duration of hospital stay, clinical symptoms of the disease exacerbation manifesting with three syndromes: pain, dyspepsia, asthenovegetative syndrome.

RESULTS: The shortest heeling of ulcer defect was achieved in the patients treated with ILRB+ER combination: 17.8 +/- 0.8 and 19.3 +/- 3.4 days for gastric and duodenal ulcer, respectively (p < 0.05 and p < 0.01 vs control).

CONCLUSION: Combined treatment of gastric and duodenal ulcer with inclusion of laser radiation (ILRB, ER, ILRB+ER) is effective in therapy of ulcer.

The influence of low-intensive laser therapy on the aggregation properties of thrombocytes in patients with peptic ulcer

The purpose of the study was to evaluate the influence of low-intensive laser therapy (LILT) on the aggregation properties of thrombocytes in patients with exacerbation of peptic ulcer (PU). The subjects, 111 patients aged 18 to 63, were divided into two groups: the main group (n = 81), and the control group (n = 30). In addition there were 15 healthy people who also underwent examination. Patients in the main group received complex treatment with untiulcer drugs and different methods of laser therapy: intravenous laser irradiation of blood, cutaneous irradiation, and a combination of both. The control group was treated with drugs only. The study found various changes in the aggregation properties of thrombocytes in patients with PU exacerbation, which consisted mostly in hyperaggregation. LILT had a normalizing effect on the aggregation properties of thrombocytes in patients of the main group.

The study revealed shortening of the time to ulcer cicatrization in patients who were treated with laser emission of the organospecific iridic zones and immunocompetent body parts in addition to standard drug therapy.

Laser iridotherapy was carried out simultaneously with irradiation of immuno-competent zones on the integument of the human body in patients with stomach and duodenal ulcers. As a result of such treatment, it was discovered that laser therapy has immunomodulating action leading to the reduction of the ulcer cicatrisation period.

The aim of our investigation was to study the influence of low-intensity laser therapy at the immunologic parameters and Helicobacter pylori (HP) microbial contamination in patients with exacerbation of peptic and duodenal ulcer. 100 patients aged 18-65 were examined and divided into 2 groups. The experimental group (75 patients) underwent a complex drug and laser therapy of various therapeutic techniques. The control group (25 patients) underwent only drug therapy. The results of this study show the immunocorrective effect of different methods of low-intensity laser therapy in the exacerbation period and more essential decrease of HP microbial contamination in the experimental group.

Registration of surface fluorescence of endogenous porphyrins during low-level laser therapy of gastric and duodenal ulcers

With the use of the developed opto-fiber spectroscope and gastroscope, the intensity of fluorescence from the ulcer surface into the gastrointestinal tract was measured in the range of 670-690 nm under the He-Ne laser (= 632.8 nm) illumination. Surface fluorescence data were collected with the use of a special diagnostic fiber with a total diameter of about 2 mm, which combines some illuminating and receiving fibers. To do the measurements, the diagnostic fiber was brought into a slight contact with the mucosa surface. The quantitative results were calculated with the use of the special fluorescent contrast coefficient (Kf), which takes into account the backscattered light as well. After the fluorescent diagnostic procedure, the low-level laser therapy was applied to all patients. It was found that: first of all, during the laser medical cure the registered fluorescence for the patients with a good dynamics of treatment had an evident tendency to a decrease, and the coefficient Kf for observed ulcers tended to approach the value obtained from normal mucosa (Kf), which indicates the normalization of the porphyrin content in the tissue. Second, the retrospective analysis of the efficacy of laser therapy versus initial values of Kf showed that, for the patients with the initial values Kf = 0.2, the efficiency of the laser treatment was the highest: the relation between the patients with good treatment results and bad ones was 4:1 (more than 80%). For the initial Kf = 0.2-0.3, this relation was 3:1 but for Kf = 0.4-0.5 it was 2:3. The differences in the efficiency of the low-level laser therapy, except the cases of the cancer in the ulcers, may be due to different content of porphyrin in the tissue: at high concentrations of the photosensitizer in the ulcer, the effect of the overdosing could be realized. In this case, another technology of laser illumination is needed. The results are discussed in terms of the free-radical conception of the stimulating effect of laser radiation.

Effect of impulse infrared laser radiation on bioenergetic metabolism in gastric mucosa in patients with gastroduodenal ulcer

Gastric mucosa and blood plasm were studied in 82 patients with gastroduodenal ulcer for content of adenylic nucleotides (AMP, ADP and ATP), histamine, amino acids. It was found that the above nucleotides occurred in the examinees in subnormal quantities. The necessary substrates, aspartate in particular, were deficient. This may lead to impairment of biosynthetic processes, to imbalance of defense factors. Photon therapy of gastroduodenal ulcer corrects bioenergetic metabolism in gastric mucosa thus producing a marked therapeutic effect.

Laser therapy of duodenal ulcers: effect on indices of microcirculation, cell membrane permeability and homeostasis of trace elements

123 patients with duodenal ulcer were examined. Laser therapy (LT) was included in the complex therapy for 89 of the patients. 34 patients underwent only traditional drug therapy and formed a control group. LT was carried out by 2 ways: intracavitary – by ulcer defect irradiation with the light guide through the gastroscope biopsy canal and helium-neon laser (HNL), and non-invasive–through the front abdominal wall in the corresponding areas, which was made by infrared laser (IRL). The comparison of two groups established that by including LT in the complex therapy for DU patients, the dynamics of clinical and laboratorial data is essential better than during traditional drug treatment (TMT). It was established that the inclusion of LT in the duodenal ulcer treatment has a positive impact on the course of the disease. This impact was displayed as faster reverse development of clinical symptoms, positive laboratory and instrumental data dynamics, blood serum microelements content normalization, membrane permeability increase by indicators of Na-Li counter-transport in the erythrocytes membrane and microcirculation improvement, mainly in the intravascular section.

Therapeutic efficiency of a combined use of low-intensity laser radiation and actovegin in gastroduodenal ulcers with inhibited cicatrix formation

AIM: To validate use of intravenous laser blood irradiation (ILBI) combined with actovegin administration in indolent gastroduodenal ulcers.

MATERIAL AND METHODS: Modern endoscopic, morphological, device, biochemical techniques and radioimmunoassay were used in examination of 92 patients with indolent gastroduodenal ulcers aged 24 to 69 years. ILBI plus actovegin was given in failure of standard medicinal therapy.

RESULTS: ILBI plus actovegin combination produced marked analgetic, anti-inflammatory and detoxication effects. Favourable trends were observed in the composition of gastric mucus, detoxication, reparative and metabolic processes in the gastroduodenal mucosa, neurohumoral regulation.

CONCLUSION: Combination of ILBI with actovegin proved highly effective in indolent gastroduodenal ulcers.

Changes in local humoral immunity during stomach ulcer healing with laser and drug therapy

The increase of the contents of serum IgA, IgG, IgM on the edge of long non-cicatrizing gastric ulcer was determined. At the same time the level of SIgA decreased in comparison with unaltered gastric mucosa. It is shown that the immunoglobulin content in the healed ulcer cicatrice depended on treatment method. The medicamentous therapy was accompanied by lowering of the contents of IgG, IgA, SIgA and by some increase of IgM level. The laser therapy is characterized by lowering of IgA, IgG, IgM level on a background of significant increase of SIgA content. The local humoral immunity after laser therapy was the same as in persons with unaltered gastric mucosa.

Insufficiant effectiveness of modem pharmacological means of duodenal ulcer (DU) treatment, increasing of complications and relapses stimulates a search of new effective methods.

We used different methods of low intensive laser radiation: laser acupuncture (LA), transdermic, infra- and overvascular methods. 280 patients with different forms and courses of ulcer disease (UD) were examined in hospital and polyclinic.

The following examination methods were used: roentgenologicai, fibrogastroscoimages with determination of H pylori, electrogastromiographic, intragastric pH-metry, perioxidate lipoid oxidation, vegetative status determination. We worked out criteria of selection of patients with UD for various treatment methods. In non-complicated and first-revealed forms of DU monotherapy by LA of skin points of activation by helium-neon laser (HNL) ULF-01 “Yagoda” and LG-75 with wave length equal to 0,63 mm was used in compari-son with traditional medicamental therapy (first group). The second group were patients with recurring of DU and accompaning diseases of digestive organs, they were treated by infrared laser “Uzor” and “Mustang” with wave length of 0,89 mm skin acupuncture ac-cording to our method in comparison with traditional medicamental therapy. In the third group of patients with long-term non-scamng DU and double ulcers the treatment was ful-filled by intra- and overvascular irradiation by HNL “Alok-1? and “Mustang”.

Good direct and far results were observed. The effectiveness of treatment in the first group was 92%, in the second – 88%, in the third – 76%.

The demand of a more effective treatment of gastroduodenal ulcer makes it necessary to combine various techniques. The last few years showed thatlasertherapy was a widely ac-knowledged means for treatment of ulcerative disease of the stomach and duodenum. Starting from 1985 we have treated 359 patients using our own techniques. In the group there were 153 female and 206 male at the age from 19 to 67. All the patients had stom-ach ulcer or duodenum when checked by gastroduodenoscopy, 21 patient had “mirror” ulcer of duodenum. Along with traditional methods of complex treatment we used low-power lasers on arcenide gallium IR-radiation (“Uzor” and “Mustang” apparatus, with the wave length of 0,89 nm, frequency of 80-3000 Hz, and pulse power of 3 to 10 W). The treatment was carried out from the moment the ulcer had been identified and in the case of bleeding-on the day it had been stopped in case of stomach ulcer-after gistology test results. Daily radiation was performed on biologically active points and on the most pain-ful points. The number of sessions was 8 to 12 depending on the size of ulcer. As to 17 patients with nonseaming callous ulcer treating was by means of laser on copper vapour. The average bedding day in the group was 17±2,76 which made 1/3 less compared to that in the control group. So, we can draw a conclusion that complex method in treatment of gastroduodenal ulcer using laser therapy is a promising technique besides it can be used both in hospital and in out patient units.

The efficiency of laser therapy in complex treatment of duodenal ulcer has been evaluated for 110 patients. Most patients (71,4 percent) were people under 40 years of age with chronic ulcerous process (89,6 percent), pronounced pain, and an ulcerous defect measur-ing 1,0 by 0,7-0,8 cm. A soft infrared laser was used. It was applied in pulsed operation at 90 Hz, 20 mW. The duration of one exposure was 4 minutes. Exposure zones varied every day: projection of the ulcer on the front abdominal wall and the point under xiphoid process of sternum were alternated with paravertebral zones along intercostal areas from ??6 to Th9 symmetrically on both sides. The cource of treatment included 10 daily pro-cedures. The technique was suggested by the Department of Physiotherapy and Balneol-ogy of St. Petersburg Medical Academy of Post-Graduate Education. The treatment effi-ciency was evaluated by means of fibrogastroduodenoscopy. The formation of paunch on the site of ulcer defect as a result of laser therapy was observed for 64 patients (58,2 percent); 18 patients (16,4 percent) showed a decrease of the ulcer defect by 80 percent. Pain was usually eliminated after the third or fourth procedure. The results obtained seem to indicate that the technique employed is quite efficient and can be used in complex treatment of duodenal ulcer.

The patients with ulcer of bulbous duodenal were examined on index of cellular and hu-moral immunity after the treatment with the help of semiconductor laser of near infra-red-spectrum for choosing the right time of setting and the dose of laser therapy as the immu-notherapy. The blood of 20 patients with ulcer bulbous duodenal was analyzed before and after the laser-therapy on the base of threefold therapy (blocs of proton pump + metroni-dazol + amoxycillin).The following indices were determined: the level of T-lymphocytes and their subpopulations, B-lymphocytes, Ig A, G, M and leukocytes, lymphocytes, seg-mental neutrophyles of peripheral blood. The laser therapy was conducted by transcuta-neous method with the help of “Uley-2k” (wavelength – 0,89 nm, pulse regime) with magnetic nozzle MN-1-055 (32 mT). Before the treatment most of the patients had the low indices of leukocytes and lymphocytes, CD-4, B-lymphocytes, Ig A, M, G with in-crease CD-8, T-lymphocytes active forms (CD-4/ CD-8 = 3,75). After two weeks of com-plex treatment with the help of magnetic laser therapy there was a cicatrizing effect of ul-cer defect most of 18 patients (90%). The laboratory results are: indices of leukocytes of blood became normal; lymphocytes and segmental neutrophyles increased with activation their function; T-lymphocytes, CD-4, Ig A, M – increased: CD-8, active forms of T-lymphocytes, B-lymphocytes became normal. Remained the low level of Ig G. So the complex treatment of ulcer of bulbous duodenal disease with the help of magnetolaser therapy brings improved tendency of disease, the fast ulcer cicatrize and immunomodula-tion effect. Therefore it must be applied on early stage of disease.

Lately various methods of laser therapy of gastric (GU) and duodenal (DU) ulcer such as transendoscoimages, intravascular, transdermic, laser acupuncture (LA) are widely used to-gether with medicaments therapy. At the same time there are no clear criteria of patient’s selection for every treatment method. The aim of this study is to examine effectiveness of various methods of laser influence on patients with relapsing form ofGU and DU. 262 pa-tients with different forms and courses of ulcer disease have been observed. The state ofantioxidant and immune systems and vegetative status have been studied by roentge-nological, fibrogastro duodenoscoimages, clectrogastromiographic and intragastric pH – metre methods. In the I group of patients with non-complicated and first revealed forms of DU monotherapy by LA according to our method of skin points activation by helium-neon la-ser (HNL) ULF-01 “Yagoda” and LG-75 with wavelength equal to 0,63 mm, 10 seances. The II group of patients with long-term non-scarring GU and DU have treated by intra-vascular blood irradiation by HNL “ALOK-I” up to 30 min,10 seances together with the use of cholinolytes and antacid’s. The III group of patients with recurring DU and pathol-ogy of gastroduodenal tract and cardiovascular system have been treated by infrared laser “UZOR” skin acupuncture to epigastric area and by supravenous acupuncture to ulnar flexion according to our method, 10 seances. Good direct and far results of LA method both in hospital and polyclinic. The effectiveness of treatment in the I group is 92,3%, II-78%, III – 88%.

Biogenic amines and cyclic nucleotides in the laser therapy of long-term nonhealing stomach ulcers

A study is presented of the effect of copper vapors laser therapy on the content of biogenic amines–serotonin and histamine and the state of adenylcyclase (AC) system (content of cAMP, cGMP and AC activity) at the edge of the gastric ulcer. Direct effect of laser radiation (single dose 10–15 J.) produced a significant increase of serotonin, histamine, cAMP, AC activity and an insignificant increase of cGMP. Healing of the ulcerative defect after 5–6 laser therapy sessions was followed by a reduction of the content of serotonin, increase of histamine, cAMP and AC activity. The authors discuss the biostimulating effect of laser radiation by influencing the inflammatory-proliferative processes in the epitheliocytes in prolonged nonhealing gastric ulcers.

Effect of laser irradiation of different intensities on the processes of healing of stomach ulcers

Patients with stubborn peptic ulcers were exposed to copper vapor laser of various intensities with similar irradiation energy. The best therapeutic effect (more rapid ulcer healing) was achieved when ulcer bottom and edges were exposed to laser of higher intensity; the effect is explained by a higher stimulation of inflammation and a more rapid and clear-cut restructuring of connective-tissue stroma on ulcer bottom.

Compared to conventional treatment, laser therapy of ulcer has a number of advantages allowing its wide introduction in both hospital and outpatient settings. Benefits and drawbacks of endoscopic and transcutaneous laser therapy are comparatively assessed.

Endoscopic laser therapy of stomach and duodenal ulcers

The authors analyse the results of the treatment of peptic ulcer of the stomach and duodenum by indestructible red laser radiation in 65 patients. Complete healing of ulcer was attained in 92.3% of the cases. In 7.7% of the patients, ulcer appeared resistant to laser radiation. Peptic ulcer relapsed in 6.2% of the patients. It is shown that the efficacy of laser radiation treatment is dependent on the ulcer localization.
